Could be a condition dropsy otherwise known as bloat, usually it’s caused by stress from the environment or some other underlying health condition. He lacks other symptoms of it though like raised scales so I doubt it’s that
I’d recommend checking water parameters and check the tank for parasites but really he might just be fat and constipated. How often do you feed? Is he swimming normally and how old is he?
