Have had him for over 1.5 years , never any issues , tank is very healthy and his tank mates are Friendly. Only difference I’ve made is adding those floating plants 2 weeks ago

That betta looks a tad overweight. If the floating plants are stopping your betta from gulping air that can be bad. Bettas have a labyrinth organ that allows them to gulp air. They need to be able to do this especially if they are on the chubby side.
