Not enough line tension while doing the wraps. Need to gradually tighten the tag ends too.

Your wraps are locking in the wrong direction, this usually happens towards the start of your wraps in my experience. Youre not doing the wraps wrong, but the previous wrap can “flip” when doing the next which will cause the knot to fail.
After i make my first wrap, i pinch the knot itself and do the rest of the wraps to insure theyre sitting proper and havent had a failed knot since. Give it a try 👍

Not securing the wraps tight enough after every loop makes this happen. It’s honestly not entirely your fault as this happens on lighter line/expensive quality leader lines. Funny enough, you’ll rarely come across this problem on heavier lines.
