Why does this happen? Is it because the line is too loose so it over wraps itself? If that's the case how do you prevent that when fishing, cause when you feel it's not always as tight as when you spool it on.

It’s most commonly, almost always, caused by reeling in slack line that overruns itself on the spool. You may be starting your retrieve too early, reeling in line that has been picked up by water flow or wind. It can also happen if you’re not closing the bail by hand.

It can happen often if you’re casting lures lighter than what your set up can do, line that’s too heavy for certain applications will cause these types of problems as well.
