Looks like dropsy, aka fish kidney failure. You can see his belly pineconing and it looks like it’s in a late stage of it. Fatility rate of dropsy is very high so I’m sorry to say he might not make it. You can absolutely try to move him to a hospital tank and try using aquarium salt but it might be too late.
